It is incredibly important that parents do not sign the school or day care
provided waiver/exemption form provided by the health department.
The language of the form is admission
by the parent(s) of negligence because they are well aware of the risks posed by their
decision:
"I hereby request an
exemption from the immunization requirements for the child listed below, have received
information about immunization and understand the risks and
possible outcomes of this decision."
Any signed document becomes a legal document. For parents that do
sign the school waiver for religous or philosophical beliefs, the signature from a
physician is not needed.
Only
upon their educated refusal
are parents offered the opportunity to exempt from vaccinations. It is unfortunate that this is only afforded to
parents on the basis that they sign a waiver which admits to negligence. You do not have to be one of those.

statutory laws at the National Vaccine Information Center. Depending on what
your state laws are, you may want to also search here for forms. For those states that have philosophical
exemption laws the below general waiver form is available to print, fill, sign and submit
to your school(s). This form can also be used for childcare. The school
administration and nurse's office will be well educated with this form.
