This statutory law
applies to all school districts
in the state of
Arizona
You can get a copy at your local school
district office.
If your child never finished the
vaccination schedule or has never been vaccinated, parents are not obligated
to even show any vaccination record to the
school nurse or school administrator. Ask for the waiver which every
school should have available for parents to
sign. Otherwise, you can also get the form at your local health department
