Below is the listing of deaths during vaccine trials [taken from the vaccine package inserts].  One would understandably assume that deaths associated with a vaccine after licensure,    must have occurred prior to licensure even though package inserts make no mention of it. 

Because multi-vaccinations in one pediatric visit is widely common, it is very hard to know what ONE vaccine may have been involved (when looking at the VAERS database).  Though some reported deaths appear to have had only one vaccine involved some, if not many, of the subjects received other vaccines days or weeks prior.

One needs to be reminded that [by FDA admission] less than 1% of all reported events are ever added to the database AND that reporting is strictly voluntary.
